BENIGN PROSTATIC HYPERTROPHY: ETIOLOGY, PATHOGENESIS AND THERAPEUTIC CONSEQUENCES

Due to the higher life and health expectancy in modern societies, the quality of life (QoL) became a very important issue in benign prostatic hyperplasia or syndrome (BPH or BPS)

Population. It is one of the major concepts identified by patients to be important in relation to sexuality before and after BPH therapy. Experts expect a drastically higher increase of elderly population within the next two decades   

We obviously identify -according to the 12 known studies and to our experience in terms of BPH and sexuality- a higher correlation between clinically manifested BPH and erectile/ ejaculatory disorders (ALF ONE & MSAM-7 studies…). These studies have more a demographic and statistical characters regarding to this concern. We were more concerned in our research group to study the  impact on sexuality in this population not only as  the result of impaired QoL due to voiding disturbances or nocturia (tiredness, lack of sleep etc…) but also to look for the reasons in the Microanatomy and Histo-chemistry.

The hormonal imbalance (aging related Lack of Testosterone and DHT) can give a chance for “ Aging Changes “ in the whole male organism including the tissues of Prostate and Corpus cavernosum stromas. The connective collagen stroma will increase, enhancing the compartments of Alpha-1 Adrenocepros 1 & 2.

Furthermore the impact on sexuality resulting from the various treatment modalities of BPH, either medical, surgical or instrumental has been often neglected in the past and not sufficiently investigated. We also try in this presentation to update the influence on sexual function of current treatments for the symptomatic BPH.
