Abstract: Testosterone treatment has been used for over 60 years in the treatment of symptoms of the andropause, including erectile dysfunction, with varying degrees of success. The studies reported here were undertaken as part of a retrospective audit of 1,000 men andropausal men treated for up to 10 years with testosterone, to establish how the results of treatment might be improved by the addition of sildenafil (Viagra) or danazol (Danol).
1,000 patients were treated with testosterone alone, in the form of testosterone undecanoate (Restandol) or mesterolone (Pro-Viron), or with testosterone pellet implants (Organon). Of the 85% of men presenting with erectile dysfunction, and 79% presenting with reduced libido, testosterone improved the ability to obtain and maintain an erection in 65% and 52% respectively, and libido increased in 69% of cases, while also producing significant and sustained relief of all the other symptoms of the andropause. Of the 100 men treated with sildenafil in combination with testosterone, erectile function was restored in 98% and libido in 95%.
The most marked change observed when danazol was added to the three treatment regimes was in the reduction of Sex Hormone Binding Globulin, which usually started above the reference range, and ended at the lower end of it. Those who needed testosterone implants and Danol to obtain a satisfactory clinical response were observed to have initially much the highest SHBG levels. It was concluded that danazol could reduce SHBG to youthful levels, and thus raise the proportion of free, biologically active testosterone, greatly enhancing the effectiveness of testosterone treatment.
